The under-eye is usually the first area of the face to show its age, and it does so through a combination of changes: bone receding at the orbital rim, the fat pad that cushioned it sliding downward, collagen falling, and skin that was already the thinnest on the body getting thinner still. The groove this leaves is called tear trough deformity, and what you see in the mirror is the shadow it casts.

The complication is that not all darkness under the eye is a shadow. Pigmentation in the skin produces the same visual result and can mimic or exaggerate genuine hollowing. Thin skin showing the blue of the vessels beneath it does too. And volume loss above the eye, in the upper socket, makes the whole area look more skeletal, which makes the hollow below look deeper than it is.

Four different causes, one appearance. Filler corrects a hollow and does nothing for pigment; used in the wrong under-eye it adds weight and puffiness and makes the area look worse. Working out which of the four you have, and in what proportion, is the entire job and it is why this area is assessed rather than simply treated.

The different kinds of under-eye darkness

A shadow cast by a hollow

The groove itself is the problem and the darkness is a trick of the light. This is the presentation that responds to filler, and it is the one filler is actually for.

Pigmentation in the skin

Genuine darkening of the skin, often more obvious in deeper skin tones and after inflammation. Adding volume does nothing here and can make the area look heavier.

Thin skin showing what is underneath

A bluish look from vessels visible through skin that has become translucent. Thickening the skin is the approach, which is a regenerative job rather than a filling one.

Puffiness rather than hollowness

Fluid retention or prolapsed fat sitting forward. Filler here is the classic mistake and reliably makes it worse. Sometimes the honest answer is surgical.

Crepey, finely lined under-eye skin

Texture rather than colour or volume, best addressed by improving the skin itself with polynucleotides or PDO mono threads.

Hollowing above the eye

Volume loss in the upper socket, which makes the whole eye look sunken. Treating above the eye is often what makes below the eye look right.

What causes dark circles and under-eye hollows?

  • Bone loss at the orbital rim with age, which deepens the groove the shadow falls into

  • Fat pads shifting downward, so the cushion that smoothed the transition to the cheek moves away

  • Thinning skin and falling collagen, which makes everything underneath more visible

  • Hyperpigmentation in the under-eye skin, which reads as darkness and mimics hollowing

  • Inherited anatomy: some families hollow under the eye in their twenties

  • Lifestyle factors including poor sleep, dehydration, alcohol, smoking and allergies, which worsen all of the above

How do I know if my dark circles are shadows or pigmentation?

A simple check: look in the mirror with light from directly in front, then from above. A shadow from a hollow changes noticeably with the light; pigmentation in the skin stays the same. Dr Toro confirms this properly at consultation, because the answer decides whether filler will help you or not.

Will under-eye filler make me look less tired?

If your darkness is a shadow cast by a hollow, yes, and often dramatically. If it is pigment, thin skin or puffiness, filler will not help and may make the area look heavier or puffier. This is the most common way under-eye treatment goes wrong and it is entirely avoidable with proper assessment.

I have had hollows under my eyes since my twenties. Is that normal?

Yes, and it is usually inherited. Some families have a shallower orbital rim or less fat in that compartment, so the groove is there long before age has anything to do with it. It is treated the same way, and it tends to respond well because the skin quality is still good.

Is there anything that is not a treatment?

Sleep, hydration, reducing alcohol, treating allergies and stopping smoking all make a genuine difference to how the area looks, particularly with puffiness. Dr Toro will mention those before suggesting anything is injected, because for some patients they are most of the answer.

What if my problem is bags rather than hollows?

Then filler is probably not the answer. Prolapsed fat sitting forward is a structural problem and adding volume around it makes it more obvious. Where that is the case, Dr Toro will say so and explain what a surgical opinion would involve rather than treating you anyway.
