Why your face reads tired

Almost nobody books an appointment because of a specific line. They book because someone asked whether they were sleeping properly, or because a photograph caught them looking older or flatter or sadder than they feel. Looking tired is the most common complaint in aesthetic medicine and the least specific.

What produces it is usually some combination of five things. A hollow under the eye casting a shadow. Pigmentation in thin under-eye skin. Volume that has slipped from the cheek, so the face reads flat and the mouth corners turn down. Skin that has lost hydration and reflects light unevenly. And the brow sitting lower than it used to, which closes the eye slightly and makes the whole face look heavy.

Each of those needs a different treatment, and treating the wrong one is why people end up looking treated rather than rested. Dr Toro’s starting point is to identify which apply to you, in what order they matter, and which of them are not worth treating at all.

The different ways a face looks tired

Tired around the eyes

Shadows, hollows, dark circles and fine lines. This is the most common presentation and the one with the most possible causes, which is why the under-eye is assessed on its own terms.

Tired in the mid-face

Flatness where there used to be a curve across the cheekbone, deeper folds from nose to mouth, and mouth corners that sit lower at rest.

Tired skin rather than a tired face

Dullness, dryness and a rougher texture, with no change in structure at all. The face is fine; the skin has stopped reflecting light well.

Tired and heavy through the brow

A brow that has dropped makes the eye look smaller and the expression flatter. It is often read as looking cross rather than tired, which is its own problem.

Tired after a period of real strain

Illness, poor sleep, stress or weight change all show in the face. Sometimes the honest advice is to treat the cause and reassess in a few months.

What makes a face look tired?

  • Hollowing under the eye, which casts a shadow no amount of sleep or concealer removes

  • Pigmentation in the thin skin below the eye, which reads as darkness rather than depth

  • Volume loss in the cheek, which flattens the mid-face and turns the mouth corners down

  • Skin that has lost hydration and collagen, so it looks dull and catches light unevenly

  • A brow that has descended, closing the eye slightly and making the upper face look heavy

  • Jaw and temple tension from clenching, which changes how the whole face holds itself

Why do I look tired when I am not?

Because the face signals rest through shadow, light and proportion rather than through how you feel. A hollow under the eye casts a shadow, flatter cheeks lengthen the mid-face, and drier skin reflects light unevenly. All three read as tired to anyone looking at you, regardless of how you slept.

Will filler under my eyes fix it?

Only if the cause is a hollow. If the darkness is pigmentation in the skin, or the issue is really a flat cheek below it, filler in the tear trough will not help and can make the area look heavier. Dr Toro works out which one applies before treating, which is why the assessment matters more than the product.

What is the smallest thing I could do?

Often a single treatment aimed at the right cause, rather than several aimed at the wrong ones. For many patients that is one area of anti-wrinkle treatment or a course of Profhilo. Dr Toro will tell you the minimum that would make a difference, and whether it is worth doing at all.

Could this be something other than ageing?

Sometimes. Poor sleep, low iron, thyroid problems, allergies, alcohol and significant stress all change how the face looks, and several of those are worth addressing before spending money on treatment. If something in your history suggests a medical cause, Dr Toro will say so and suggest you investigate it first.

How long before I look less tired?

It depends what is being treated. Filler and under-eye treatment change things the same day, settling by two weeks. Anti-wrinkle treatment takes about two weeks to reach full effect. Skin treatments such as Profhilo and polynucleotides work over two to three months, because they rebuild rather than replace.
