Loss of Firmness and Facial Definition in Marylebone, London

Treatment for jowls, a blurred jawline and a face that has lost its structure, at Dr. Toro Aesthetics in Marylebone and St John’s Wood. Volume, lift and skin quality are three different problems with three different answers.

Loss of Firmness & Facial Definition

Why a face loses its structure

A young face is a triangle with the wide edge at the cheekbones. An older face inverts it: weight moves downward, the cheek flattens, the jawline blurs and the widest part ends up at the jaw. Almost everything people dislike about how their face is ageing is a version of that one change.

Three things drive it and they happen at once. Facial fat sits in discrete compartments that shrink and slide downward, so volume leaves the upper face and gathers at the lower. Bone recedes at the eye socket, the mid-face and the jaw, removing the scaffolding the soft tissue sat on. And collagen and elastin fall, so skin that used to snap back simply drapes.

Treating that means deciding which of the three is doing the most in your face. Replacing volume high in the cheek supports everything below it. Repositioning tissue with threads addresses descent directly. Improving skin quality changes how well any of it holds. Dr Toro will also tell you when the descent is beyond what non-surgical treatment can fix, which is a conversation worth having early rather than after you have spent thousands.

How lost definition usually shows up

Jowls forming along the jawline

Tissue that has descended past the jaw, breaking what used to be a continuous line from ear to chin. The single most common reason people ask about lifting treatments.

A flattened cheek

The curve across the cheekbone gone, so the face reads flat in photographs and light falls differently across it.

Deeper folds from nose to mouth

Usually a mid-face that has dropped rather than a crease that needs filling. Treating the fold directly is the classic mistake.

A jawline that has lost its edge

Where the face used to end and the neck used to begin has become a gradient rather than a line.

Marionette lines and downturned mouth corners

A mouth that looks unhappy at rest, which is descent plus muscle pull rather than an expression you are actually making.

Crepey skin rather than sagging

Where nothing has descended much but the skin has thinned and lost its bounce. This needs skin treatment, not lifting.

What causes loss of firmness and definition?

  • Facial fat compartments shrinking and sliding downward, moving volume from the cheek to the jawline

  • Bone loss at the eye socket, mid-face and jaw, which removes the support the soft tissue sat on

  • Collagen and elastin declining from the late twenties, so skin drapes rather than recoiling

  • Cumulative sun exposure, which degrades elastin and is the largest single modifiable factor

  • Significant or rapid weight loss, which empties the face faster than the skin can adapt

  • Genetics and facial structure, which decide how early descent shows and how visibly

Frequently asked
questions

What is the best treatment for jowls?

It depends what caused them. If tissue has descended, a thread lift repositions it. If the face has hollowed above them, filler at the cheek and jaw restores the support and the jowl looks smaller as a result. Most patients need some of both, and Dr Toro will say when neither will be enough.

Can filler lift my face?

Not literally. Filler replaces volume, and replacing volume in the right place can make a face look lifted because the tissue is supported again. It does not move tissue that has descended. Where real repositioning is needed, threads or surgery are the honest answers and filler alone will disappoint you.

When is non-surgical treatment no longer enough?

When skin has lost so much elasticity that it will not hold a lift, or when there is enough descent that filling and threading would only add weight. Dr Toro will tell you when that point has been reached and what surgery would involve, rather than selling you a plan that cannot deliver.

Will treating my cheeks really help my jawline?

Often yes, which surprises people. The mid-face supports everything beneath it, so replacing volume at the cheekbone frequently improves the nasolabial fold, the marionette line and the jowl at the same time. Treating each of those directly is more expensive and usually looks heavier.

How long do the results last?

Filler in structural areas such as the chin and jaw typically lasts 12 to 18 months because those areas move little. A thread lift usually gives 12 to 18 months, with the threads dissolving at six months and the collagen carrying the rest. Skin treatments are maintained every six to nine months.
